• Tasks:
  • Provide personal care and assistance with daily living activities
  • Monitor and report patient conditions and changes
  • Assist clients with mobility, medication reminders, and hygiene needs
  • Maintain accurate patient records and documentation
  • Support clients with emotional and social needs
  • Follow individualized care plans and safety procedures
  • Communicate effectively with healthcare professionals and family members
• Skills:
  • Previous home health or caregiving experience
  • CNA, HHA, or relevant healthcare certification
  • Strong communication and interpersonal skills
  • Compassionate, patient-focused attitude
  • Reliable transportation and valid driver’s license preferred
